To help you understand how to fill these forms accurately, please read these guidelines carefully. All fields ara MANDATORY. Failed to complete this form accurately and in a timely manner, might jeopardize your participation in Bentang Jawa 2026. You may check the timeline of the registration process here.
REGISTRATION DATA VERIFICATION
To help verify that the person filling out this form is actually the person who was registering, you need to to fill out the first three fields with the correct CATEGORY, REGISTRATION CODE (you can find this in confirmation / announcement email, and EMAIL ADDRESS that was used to register.
PARTICIPANT CANDIDATE CREDENTIAL
The next seven fields are your credential as participants.
- Type of Official Identity Document. Choose KTP for Indonesian Citizen, and PASSPORT for foreign nationals. Indonesian Citizen may not use Passport for any reason, you can still find your NIK at your Family Card (Kartu Keluarga)
- Official Identity Number. Key in your identity number as stated in your official identity document; NIK for Indonesian and passport number for foreign nationals.
- Full Name. Key in your full name EXACTLY AS STATED in your official identity document. No more, no less.
- Date of Birth.
- Blood Type.
- Privy ID. Bentang Jawa requires every participant to have a Privy account (Privy ID) for valid and accurate digital waiver signing without the need for in-person meetings or physical documents, while eliminating the need for printing, making it more environmentally friendly. Please create a Privy account through the Privy app if you and/or your Next of kin don’t already have one.
- Privy Email Address. Key in your email address that was used when creating your Privy account.
NEXT OF KIN CREDENTIAL
The next six fields are your next of kin credential. Your next of kin will also serve as your emergency contact through out the event. Ideally, your next of kin and/or emergency contact, are a person who have the authority and capability to make important and crucial decision on behalf of yourself, should there are medical emergency situation arise. Please keep this in mind when filling this part of the form.
- Full Name of Next of Kin. Complete name of your next of kin, no nickname.
- Relationships to Participant. What is their relationship to you?
- Mobile Number of Next of Kin. Key in your next of kin active mobile number and/or WhatsApp number.
- Email Address of Next of Kin. Key in your next of kin active email address.
- Privy ID of Next of Kin. Bentang Jawa requires every participant to have a Privy account (Privy ID) for valid and accurate digital waiver signing without the need for in-person meetings or physical documents, while eliminating the need for printing, making it more environmentally friendly. Please create a Privy account through the Privy app if you and/or your Next of kin don’t already have one.
- Privy Email Address. Key in email address that was used by your next of kin when creating their Privy account.
Special note for international participants / next of kin regarding Privy account creation. Foreign nationals are able to create a Privy account, but most of them cannot complete further verification due to lack of supporting documents. This is normal, and at this phase, your Privy account is already created and can be used for electronic approval of a document. You can find your Privy ID on your account profile.
